Thank you for your interest in speech-language therapy services for your child. At your first appointment, you will be asked to provide a copy of your insurance card and driver's license, and you will complete a Clinical Billing Practices, Procedures, and Policies form with a staff member and/or your therapist. Prior to your first appointment, please complete the following forms:

A child's health is important to the therapeutic process. Typically therapy includes a lot of physical activities. If a parent feels their child cannot participate fully in therapy, then a child should remain home. If a child shows signs of illness, parents are asked not to bring them to therapy. Parents can often tell when a child is getting sick through such signs as paleness, irritability, lethargy, or loss of appetite. It is helpful to keep a child home in these situations. Therabill is our confidential online patient portal. You will receive appointment reminders through this portal and you can also use it to view schedules, documentation, and billing information. Please activate your online Therabill account as soon as you receive the link via email. Please confirm or cancel your appointments using Therabill. In order to secure your child's Protected Health Information, please communicate all information regarding your child's plan of care via direct phone contact or through the Therabill message center.
